Antenatal multicystic kidney disease

A 32 years old woman with 8 months gestation went for a regular antenatal sonography. It was found that the fetus had a right multicystic kidney disease. Other kidney was normal. There was no oligohydramnios. Other antenatal checkup and maternal history was normal.

How should this child be managed further?
Expert Opinion :
There was an antenatal diagnosis of multicystic dysplastic kidney made on ultrasound with other kidney being normal. Since the mother is already in 8 months of pregnancy, in such a case, one should just wait for the child to be delivered normally and repeat an ultrasound of abdomen in the child on Day 3 of life. If the ultrasound confirms the antenatal report, the child should be monitored with renal function tests, blood pressure and growth. Also renal scan can be done at 3 months of age to determine the function of that kidney. If there is any evidence of reflux, micturating cystourethrogram may be required.

If the other kidney is normal, there is no need for any intervention. The outcome is good.

wait to delivery at term, follow up the infant with renal tests and genetic tests, and consider right nephrectomy if need be
