Nepafenac

Mechanism :

Nepafenac is a prodrug. After penetrating the cornea, nepafenac undergoes rapid bioactivation to amfenac, which is a potent NSAID that uniformly inhibits the COX1 and COX2 activity.

Indication :

Treatment of pain and inflammation that occurs during cataract surgery

Contraindications :

Hypersensitivity, ASA or NSAID-induced asthma, Pregnancy 3rd trimester, Contact lens use.

Caution if bleeding risk, complicated or multiple ocular surgery, corneal denervation, corneal epithelial defects, dry eye syndrome, diabetes mellitus, rheumatoid arthritis.

Dosing :

>10 years or age:

0.1% or 0.3% ophthalmic suspension:

1 drop instilled in the affected eye(s) three times a day beginning 1 day prior to surgery, on the day of surgery, and for 14 days following surgery.

Adverse Effect :

More common: Blurred and decreased vision, capsular opacity, foreign body sensation, increased IOP, sticky sensation.

Less common: Itching of the eye and eye pain, photophobia, dry eye, conjunctival and corneal edema, headache, hypertension, swelling and/or redness of the eye and lining of the eyelid, vitreous detachment.

Interaction :

Difluprednate: NSAIDs may slow healing.

Hepatic Dose :

No dose adjustments are recommended.
