How to confirm a case of malaria or dengue with a widal O titre positive, whether the widal is true positive or false positive due to polyclonal B lymphocyte stimulation?

NAAT/PCR will confirm; or do a blood culture and repeat Widal titres after few days; a positive culture for typhoid or a significanlty rising titre can be considered positive

Answer 1

On clinical suspicion, check for malarial parasites and for dengue IgM and IgG. If fever subsides with antimalarials, it is unlikely to be enteric fever.
