Diagnostic Dilemma

EPITROCHEAL LYMPHADENOPATHY

Author:
Question
A 9 years old boy presented with swelling in right elbow which was increasing since past 5 months. He had been treated with antituberculous therapy {ATT} for 6 months but the swelling persisted. A biopsy was done that did not show any acid fast bacilli {AFB} and histopathology was suggestive of reactive lymphadenitis. There was no history of insertion of hand in water pools. He used to play with cats.

What is the cause of epitrocheal lymphadenopathy _?
Expert Opinion :
Since the child used to play with cats, one should rule out toxoplasmosis. The child’s toxoplasma IgM and IgG were positive by ELISA. He was treated with TMP-SMX to which his lymphadenopathy subsided.
