Portal cavernoma with pancytopenia

A 5 years old boy was detected to have extrahepatic portal hypertension due to massive splenomegaly. There was no hematemesis. Colour doppler showed portal cavernoma. Oesophageogastroscopy showed 1 small varix. He had pancytopenia. {Hemoglobin = 8.5 gm, dl, white cell count = 3700 , cumm and platelets of 81,000, cumm}. He subsequently had 2 episodes of epistaxis

When should this child undergo splenectomy with shunt surgery_?
