Aluminium Hydroxide

Mechanism :

This compound is used as an antacid. The hydroxide reacts with excess acid in the stomach, reducing its acidity. This decrease of acidity of the contents of the stomach may in turn help to relieve the symptoms of ulcers, heartburn or dyspepsia. It can also cause constipation and is therefore often used with magnesium carbonate. This compound is also used to control phosphate levels in hyperphosphatemia.


Indication :

• As an antacid.

• Hyperphosphatemia associated with chronic renal failure.


Contraindications :

Alkalosis, hypermagnesemia, where distention may be due to partial or complete intestinal obstruction. Not recommended for severely debilitated patients or those with impaired renal function.


Dosing :

Antacid:

Oral (Liquid 320 mg/5 mL): 5 to 15 mL (320 to 960 mg) every 6 hours.

Hyperphosphatemia associated with chronic renal failure:

Serum phosphorus levels >7 mg/dL:

Use only for 4 to 6 weeks. Dose should be individualized based on phosphate intake/absorption and dialysis clearance.

Mean phosphorous binding:

22.3 mg phosphorous/5 mL of aluminium hydroxide.


Adverse Effect :

Dementia, malaise, seizures, confusion, coma, hypophosphatemia, hyperalbuminemia, osteoporosis, constipation, anorexia, urinary calculi, muscle weakness, osteomalacia.


Interaction :

Phosphates: Aluminium hydroxide may by reacting with phosphates to form insoluble aluminium phosphate, cause phosphorus deficiency in those patients whose diet is low in phosphorus.
Antacids: Can interfere with the absorption of iron preparations, tetracyclines or other antibiotics.
Should not be taken within 2 hours of another medication because the effectiveness of the other medication may be altered.
