IN ONE OF MY QUERY ABOUT PACKED CELL TRANSFUSION
IN THALLESEMIC PATIENT YOU REPLIED IN SEVERLY
ANAEMIC CHILDREN LIKE WHERE Hb IS 3 WE SHOULD
GIVE PACKED CELL @ 3 CC, Kg. SIR NORMALLY PEDIATRIC
BLOOD PACK ARE AVAILABLE IN 100 ML PACK.IF CHILD IS
OF 7 Kg HE WILL BE TRANSFUSED ONLY 21 CC OF PACKED
CELL REST WILL BE WASTED SO HOW SHOULD WE
APPROACH AND MANAGE CHILD OF LESS THAN 10 Kg WEIGHT WITH LOW Hb LEVEL SO THAT BLOOD IS NOT
WASTED.AND WE TRANSFUSE MAX AMOUNT OF AVAILABLE BLOOD.

If you transfuse more quantity of blood, make sure it does not push the child into congestive cardiac failure. You can try and get aliquots of 50 cc if your blood bank can do the same for you.

I think it depends on the institute and locally prevalent practice. What we do is we calculate as per body weight, the total volume of PRBCs required. Add to it the expected volume of tubings and chamber and then raise the demand for that much volume only. Blood bank dispenses that much only so that there is no wastage. So it kind of depends on blood banks also. Blood products are a precious resource and all attempts should be made to prevent their wastage !!
