Mechanism :
The mechanism of action for Metamizole is still not clear. It is assumed that Metamizole itself as well as its metabolite (4-N-Methylaminoantipyrin) on the central nervous system (spinal cord and brain) attack, but also in the other body tissues. Both seem to inhibit the pain mediation by prostaglandins. Prostaglandins are messengers involved in the sensation of pain. Thus, the effect of Metamizole and its main metabolite would be similar to those of the non-steroidal anti-inflammatory drugs.
Indication :
Contraindications :
Hypersensitivity to the active substance, as well as similar substances from chemical group of Pyrazolone or Pyrazolidine.
Bone marrow function disorders and diseases of the haematopoietic system.
Intolerance of other analgesics (non-opioid analgesics, non-steroidal painkillers) in the form of allergic reactions after use.
A genetically induced lack of a specific enzyme, the so-called glucose-6-phosphate dehydrogenase (risk of anaemia due to destruction of blood cells).
Certain liver dysfunction (acute hepatic Porphyria).
Dosing :
10-15 mg/kg/dose every 6-8 hours orally.
Adverse Effect :
Allergic rash, low blood pressure, leukopenia, severe skin reactions (Stevens-Johnson Syndrome, Lyell Syndrome, Toxic Epidermal Necrolysis), circulatory shock, neutropenia, thrombocytopenia, renal failure, proteinuria.
Interaction :
Cyclosporine: The concentration of Cyclosporine in the blood, and thus the effectiveness of the drug can be reduced by concurrent treatment with Cyclosporine. Simultaneous use is therefore the blood Cyclosporine concentration medically to control.
Metamizole: Simultaneous use of Metamizole and the antipsychotic chlorpromazine, a severe drop in body temperature can occur.
For the Pyrazoline chemically related to Metamizole is known that enter into interactions with oral antidiabetic drugs, captopril (agents for reducing blood pressure), lithium (for depression), methotrexate (drug), and triamterene. In addition, they evoke changes the effectiveness of other antihypertensive drugs and drainage equipment (except furosemide). Can show these interactions in Metamizole.
Hepatic Dose :
Use with caution in children with liver impairment. Dose reduction may be required.
