A 4 years old boy has fever for 3-4 days then he developed episodes of convulsion, convulsion are generalized and last for 5 min followed by loss of consciousness for 10 min. in last 2-3 days he has 2 episodes of convulsion which lasted for 5 min in between the episodes pt remain normal. His CT is normal. Widal positive. He did not have any convulsions before this episode. No neck stiffness, Kernig`s negative. What could be the differential diagnosis? Typhoid encephalopathy or fever-induced epilepsy?

Expert Answer :
If a patient has a normal mental status in between seizures, less likely to have typhoid encephalopathy. Complex febrile seizures seem likely. If he is treated for typhoid and does not have further seizures, there is no need for antiepileptic therapy chronically.
