Dr. Ira Shah talks about tuberculosis treatment guidelines-practical diagnostic dilemmas in clinical practice are routinely faced as worsening of TB or paradoxical reaction.
The paradoxical phenomenon of the appearance of tuberculomas while on successful antituberculous therapy is supposed to be due to an immunological reaction. Difficult to distinguish between drug resistance and paradoxical reaction. Responds to the same ATT and may require steroids. Restarted on steroids and second-line ATT was continued. After 11 months, weight is increasing with normal milestones and on regular follow up. As physicians, our first goal should be DO NO HARM….. A close analysis may help to prevent overtreatment and under-treatment.
