Cefuroxime

Mechanism :

Cefuroxime is a semisynthetic, broad-spectrum, second generation cephalosporin antibiotic for parenteral administration.


Indication :

• Pharyngitis/Tonsillitis

• Pneumonia

• Sinusitis

• Otitis media

• Skin and soft tissue infections

• Gonorrhoea

• Osteomyelitis

• Lyme disease


Contraindications :

Cefuroxime is contraindicated in patients with known allergy to the cephalosporin group of antibiotics.


Dosing :

<3 months: Safety and efficacy not established.

>3 months-12 years: 30 mg/kg/day in 12 hourly divided doses for 10 days (Maximum: 1 g/day). Alternatively, 75-150 mg/kg/day IV/IM in 8 hourly divided doses for severe infections; do not exceed 6 g/day. Duration varies with infection type.

>12 years: 250-500 mg tablet every 12 hours for 10 days.

Neonates (off-label):

<7 days, <2 kg: 100 mg/kg/day divided 12 hourly IV.

<7 days, >2 kg: 150 mg/kg IV/IM divided 8 hourly.

>7 days: 150 mg/kg IV/IM divided 8 hourly.


Adverse Effect :

Pseudomembranous colitis, nausea and vomiting, diarrhea, rash, urticaria, angioedema, pruritus, false positive urine glucose test, positive direct Coombs tests.


Interaction :

Argatroban: Increases the level of argatroban by anticoagulation.
Bivalirudin: Increases the level of bivalirudin by anticoagulation.
Fondaparinux: Increases the level of fondaparinux by anticoagulation.
