A 10 yrs old patient is having a fever for 10 days. His widal test salm typhi o titers are 1,320. Tlc 15000. MP negative, urine nad. The patient was given inj Ceftriaxone and Azithromycin for 7 days but fever still persisting. What could be probable cause?

Does the child have hepatosplenomegaly or any clinical findings? What is the blood culture and chest Xray? Get him tested for brucellosis, and also an autoimmune disease. More details are required to comment further.

child may not have typhoid..look for other causes

widal test could be false positive. look for other possibilities like liver abscess, hidden tuberculosis, even a trial of antimalarial, etc.
