Question of the Week

I have a patient 14-year-old, 34 kg with a history of undocumented fever for 3-4 days, then he became dull, in the form he stares at the wall and doesn't like anyone to speak to him and even pushes his parents who come to approach him, he came to us Systemic exam was normal, no fever, vital normal, CNS he was conscious, oriented, no focal signs, CT head and LP are normal.
The child today is even more mute and not talking, he is just seeing here and there, while giving injection he objected by saying Hatoo. I have no clues, all CBC, KFT, and LFT are normal.
Widal shows 1, 160 O titer, and H is 1, 80, I think it's not specific, blood culture awaited. No fever since admission, not a single spike. I am thinking of a psychiatrist's opinion, or any other possibility.
